For each question, think about how things have been going over the past few weeks and slide to the number that feels most true. A 10 means thriving in this area. A 5 means somewhat thriving. A 1 means this area has felt especially hard. At the end, you’ll get a snapshot to guide your next steps.

1. Focus and Executive Function

How well are you able to stay organized, manage time, focus on tasks, and follow through on responsibilities? For adults, this might include finishing work projects, remembering appointments, or staying on top of household routines. For kids, it might include completing homework, following directions, or remembering what to bring to school.

2. Stress, Worry, and Anxiety

How often does stress, worry, or anxiety get in the way of your daily life? For adults, this might include a racing mind, trouble relaxing, or physical tension. For kids, it might show up as clinginess, trouble sleeping, stomachaches, or frequent worry about school or friends.

3. Self-Esteem

How strong is your current sense of self-worth and confidence? For adults, this might include believing you can handle what comes your way and speaking to yourself with compassion. For kids, it might show up as trying new things, being proud of their efforts, or bouncing back after mistakes.

4. Managing Emotions

How well are you able to manage strong emotions like frustration, sadness, or anger? For adults, this might include staying calm during stressful moments or recovering quickly after a hard day. For kids, it might include using calming tools, having fewer outbursts, or talking about their feelings instead of acting them out.

5. Making Friends

How are things going socially—when it comes to friendships or connection with others? For adults, this might include feeling supported by friends, enjoying social time, or maintaining close relationships. For kids, it might include being invited to play, joining group conversations, or keeping up with friendships.

6. School or Work

How well are you managing the demands of school (for kids) or work (for adults)? For adults, this might include meeting deadlines, staying engaged, or managing work-related stress. For kids, it might include completing assignments, feeling supported by teachers, or staying focused in class.

7. Tech Overuse

How balanced is your screen time—for things like gaming, YouTube, or social media? For adults, this might include knowing when to unplug, being present with loved ones, or keeping tech use aligned with your values. For kids, it might include transitioning off screens without conflict, getting enough sleep, or balancing screen time with movement and in-person connection.

8. Physical Factors

How well are your physical needs—like sleep, nutrition, movement, and medical care—being met? For adults, this might include getting enough rest, eating well, staying active, or managing chronic conditions. For kids, it might include having consistent sleep routines, regular meals, active play, or support for any health concerns.

9. Overall Thrive Score

When you take all of these areas into account, how would you rate your overall level of thriving right now? A 10 means you're thriving across most areas. A 5 means you’re doing okay but could use some support. A 1 means life feels especially hard right now.
